The physical examination shall include: Tuberculin skin testing by Mantoux method with negative results every 2 years; or, if tuberculin skin test is positive, an initial chest x-ray with results noted. Tuberculin skin testing may be completed and certified in writing by a registered nurse or a licensed practical nurse instead of a licensed physician, licensed physician's assistant or certified nurse practitioner. Tuberculin skin testing will be completed prior to employee's dates of hire. HR and the registered nurse will make the change. Changes have already been implemented. New employees beginning Monday, February 17, 2025, had a tuberculin skin test placed and read by their date of hire. Employees are unable to start employment until Tuberculin skin test has been read. Corrective actions are in the process of being written for all current employees and should be completed by 2/21/25. Individuals shall be able to evacuate the entire building, or to a fire safe area designated in writing within the past year by a fire safety expert, within 2 1/2 minutes or within the period of time specified in writing within the past year by a fire safety expert. The fire safety expert may not be an employe of the home or agency. Staff assistance shall be provided to an individual only if staff persons are always present at the home while the individual is at the home. The previous plan of correction that was approved stated we would complete a repeat drill within the same month. This violation followed our previously approved plan. Moving forward, we will complete practice drills prior to running the required monthly drill. The organization will train Home Leaders advising practice drills are to be completed prior to the required monthly drill. Home Leaders are responsible for training their team members. Training will be provided organization-wide within thirty days (3/23/2024). Practice drills will be implemented by 3/23/2024. Program Specialists will be training Home Leaders on March 11, 2024.
