On 5/8/25 at 10:10AM, there was no screen in the window in the kitchen, to the left to the refrigerator. | Windows, including windows in doors, shall be securely screened when windows or doors are open. | In each of the rooms, the kitchen and the bedroom to the left of the first-floor bathroom, in which there are not screens, there is another window that does open and the second kitchen window does have a screen, the bedroom window has a window air conditioning unit in place. The windows in each of those rooms that do not have a screen cannot be easily opened by staff or individuals. On 5/20/25 those two windows that cannot be easily opened were screwed shut so that they could not be opened because there is currently no screen available for those windows. |

| On 01/07/13 a new drawer pull was purchased by Program Supervisor Emily Keeler and installed by Program Worker Todd Johns that same day. Program Supervisor Emily Keeler will complete a monthly inspection of the home to make sure all furniture and equipment are in good repair. She will submit a Property Damage Report for repairs to the agency administrative office and repairs will be completed within two weeks or arrangements will be made to replace the item if it is not repairable. Documentation (photo) of the repair and copy of property damage report submitted. |

6400.68(b) | On 11-27-12, the temperature of the hot water in the first floor bathroom shower was 132 degrees F. Fully implemented - cs - 3/5/13. | (b) Hot water temperatures in bathtubs and showers may not exceed 120°F.
| The water temperature was adjusted immediately and was below 120 degrees before program participants were home to use hot water. Since a program participant was adjusting the temperature of the hot water tanks on his own, thinking he was being funny, a cage was built aroung both tanks to discourage tampering. All staff are to follow the agency policy on Water Temperature Monitoring which requires weekly testing and a three day procedure for monitoring temperatures of 120 degrees or more in order to keep the temperature below the limit. Program Supervisor Emily Keeler checks the Water Temperature Chart weekly and makes recommendations to staff if adjustments need to be made to maintain proper temperature. The supervisor also had a discussion with the program participant about water temperature safety and understanding when a joke or prank can be a safety hazard. Documentation of the correction (photo) and water temperature weekly measurement chart for the home submitted. |
