6400.68(b)The water temperature reading taken from the first-floor bathroom's bathtub faucet did not consistently read below 120° Fahrenheit. Although the baseline temperature was below this limit, the temperature was observed to spike above the limit approximately every 20 to 30 seconds, reaching readings of 133.5°, 120.5°, and 135° Fahrenheit for approximately 2 seconds per temperature spike. The hot water temperature exceeded the 120° Fahrenheit allowable reading during observation. Hot water temperatures in bathtubs and showers may not exceed 120°F. This cordless phone produced an error message when it was turned on and it could not be made to produce a dial-tone or make a phone call despite repeated attempts to fix the error.A home shall have an operable, noncoin-operated telephone with an outside line that is easily accessible to individuals and staff persons. 6400.72(a)Two of the windows in what the provider has designated as "Bedroom #1" were lacking window screens at the time of inspection. As there were no screens within the home that could be fit into these windows' window frames should they be open and in use, these windows were incapable of being securely screened.Windows, including windows in doors, shall be securely screened when windows or doors are open. 6400.101This home's basement consists of two rooms---a smaller room and a larger room---and an attached garage, which has an exit door that provides egress to the home's back yard. The stairs within the home lead down into the smaller room of the basement. The single door leading from the smaller room into the larger room (through which the garage is accessed) was equipped with a deadbolt lock with a keyhole side facing the smaller room. In other words, if the deadbolt lock were engaged, it would be impossible to enter the larger room through this doorway without the key to the deadbolt lock, blocking off the path of egress through the larger room and garage. As such, the configuration of the deadbolt lock on this door constituted a barrier to safe egress from the home during a fire related emergency.Stairways, halls, doorways, passageways and exits from rooms and from the building shall be unobstructed. The agency has locked the basement access all together as we do not have a use for the basement area. The individual does not have access to the basement. The portion of the basement in question was agreed prior to leasing from the owner that they wanted to retain that area for their use. 04/19/2024 Implemented
6400.110(a)There was no smoke detector found within the home's attic, which is accessible via pull-down stairs in the home's attached garage. A home shall have a minimum of one operable automatic smoke detector on each floor, including the basement and attic. The agency retrained the Residential Director with regard to measuring compliance with physical site issues in this and all agency homes specific to the measure of accessibility to crawl spaces and the installation of equipment if needed. 04/19/2024 Implemented

There was no smoke detector located within the home's attic, which is accessible via pull-down stairs in the home's attached garage. The smoke detectors found on the home's first floor and in the basement were not interconnected with each other.If the home serves four or more individuals or if the home has three or more stories including the basement and attic, there shall be at least one smoke detector on each floor interconnected and audible throughout the home or an automatic fire alarm system that is audible throughout the home. The requirement for homes with three or more stories does not apply to homes licensed in accordance with this chapter prior to November 8, 1991. 6400.111(a)There was no fire extinguisher located in the home's attic. The only fire extinguisher in the home's basement did not have a label indicating that it had a 2-A rating. This fire extinguisher appeared to be old and rusted, and it lacked an inspection tag. As such, it can be reasonably determined that the fire extinguisher located in the basement was no longer operational.There shall be at least one operable fire extinguisher with a minimum 2-A rating for each floor, including the basement and attic. The Fire Extinguisher in the basement has been removed and replaced with an extinguisher with an up to date tag and in proper working condition. 6400.111(f)There was a fire extinguisher in the home's laundry room that did not have a date of inspection located on it. A fire extinguisher shall be inspected and approved annually by a fire safety expert. The date of the inspection shall be on the extinguisher.
