A large bath towel hanging on the towel rod had an area of approximately 6 inches by six inches smeared with a light brown substance. A washcloth was found wadded and appeared to be soiled with a like substance on the shelf in the shower. The toilet seat had a small smear of the same substance on the surface. The inside of the toilet bowel had a dark smear approximately two inches by two inches that appeared to be feces on the inside of the toilet bowl.Clean conditions shall be maintained in all areas of the home.LSP cleaned the area on 1/23/25, upon return from vacation. LSP will change the towel and washcloth each day to ensure cleanliness. The toilet and surrounding areas will be checked daily and cleaned as needed. 6500.101The exit screen door from the laundry area of the downstairs of the home was obstructed by a rake that was pinned between the home and a large playhouse with the handle and head of the rake laying in front of the door. The rake was covered with snow and appeared to be frozen in place.
