6400.63(a) | On 5/21/25 at 1:29 PM, the hot water temperature measured 122.3 degrees Fahrenheit at the sink in the bathroom, across from the dining area. | Heat sources, such as hot water pipes, fixed space heaters, hot water heaters, radiators, wood and coal-burning stoves and fireplaces, exceeding 120°F that are accessible to individuals, shall be equipped with protective guards or insulation to prevent individuals from coming in contact with the heat source. | The regulation is necessary because it minimizes the risk of individuals suffering burns from contact with exposed heat sources. The violation occurred because the program director failed to conduct self-assessment checks before the inspection and did not verify that all water heaters in the home met the required regulations. Additionally, she did not follow up with the staff regarding the delegated water temperature checks.
Our contractor was unable to adjust the water temperature, as only the landlord has access to the water tank located in the basement of the first-floor apartment. Unfortunately, the landlord was on vacation, but he will adjust the temperature tomorrow since the water tank is full. A picture will be sent to the licensing supervisor once I receive it from the landlord. |

6400.70 | On 5/21/25 at 1:30 PM, the telephone in the home was inoperable. | A home shall have an operable, noncoin-operated telephone with an outside line that is easily accessible to individuals and staff persons.
| During the inspection, it was noted that the phones in the homes were not connected to the network. This violation occurred because the program director and program specialist failed to contact the agency vendor while preparing the house for licensing. Every room must have an operational phone that can be used in emergencies, for contacting family and friends, as well as for easily reaching the nearest hospital or poison control center. Xfinity connected the phones on June 6, 2025, and all phones in the home are now functional (see attached document). License supervisor may call the phone number on the attachment to verify that it is connected. |
