The back entrance had a small area of cleared snow, however the sidewalks that lead from the back landing to the side of the home were covered in snow/ice. This exit is used as a fire escape and the walkways shall be clear from snow/ice. Outside walkways shall be free from ice, snow, obstructions and other hazards. Provider agency staff cleared all walkways and driveways around the home from snow and ice. Provider agency reeducated Residential Directors and DSP's as to the regulations pertaining to Outside Walkways at all times. See submitted document. 12/31/2025 Implemented
6400.82(e)Bathtubs and showers shall have a non slip surface or mat. The basement bathroom contained a stand up tile shower which did not contain a non slip mat at the time of inspection. Bathtubs and showers shall have a nonslip surface or mat. Provider put in place a non-slip mat in the basement bathroom at this location.
