6400.63(a)At 12:28PM, the hot water temperature measured 124.3F at sink in the kitchen of the home.Heat sources, such as hot water pipes, fixed space heaters, hot water heaters, radiators, wood and coal-burning stoves and fireplaces, exceeding 120°F that are accessible to individuals, shall be equipped with protective guards or insulation to prevent individuals from coming in contact with the heat source. In order to correct this violation, Family Services United took immediate action by promptly adjusting the hot water temperature to ensure it falls within the acceptable safety range. After the adjustment, the hot water temperature was re-measured several times throughout the day to monitor consistency and assess whether the hot water tank settings fluctuate over the course of the day. At 12:23PM, an uncovered, plastic cup with half-filled with a blue liquid was on the top shelf in the refrigerator in the kitchen of the home. [Repeat Violation, 3/5/2024, 7/17/2024]Food shall be protected from contamination while being stored, prepared, transported and served. In order to correct these violations, Family Services United took the following immediate actions. The gallon of milk was promptly removed and discarded from the refrigerator. The uncovered plastic cup with blue liquid on the top shelf of the refrigerator was also removed and disposed. Staff were immediately reminded of the importance of regularly checking for expired items and properly covering all food and beverages stored in the refrigerator. 10/08/2024 Implemented

S. § § 1001¿1080) shall submit an application for a certificate of compliance prior to commencing operation of the facility or agency and may not commence operation until notified that a certificate of compliance will be issued.In order to correct this violation, Family Services United conducted an internal review to identify gaps in the compliance process that led to the commencement of operations without obtaining prior approval. On 03/07/2024 all poisonous materials were removed from the bathroom, kitchen, and all areas of the home where accessible to individuals. As of 03/07/2024 all poisonous materials are and will be stored in the kitchen cabinet with Cabinet Handle Locks, inaccessible to individuals. Individual #1 assessment and ISP have conflicting information regarding the individual¿s ability to use poisons in a safe manner.
