If a medication is prescribed to treat symptoms of a diagnosed psychiatric illness, there shall be a review with documentation by a licensed physician at least every 3 months that includes the reason for prescribing the medication, the need to continue the medication and the necessary dosage.A new position at Emmaus Community of Pittsburgh of a Medication Compliance Coordinator (MCC) was started in 05/2018. Angela "Angie" Monk was the successful candidate to take on this new position within Emmaus Community of Pittsburgh. Starting 9/7/18, Medication Compliance Coordinator (MCC), Angie Monk, created a new spreadsheet in order to track all 90 day Psychiatric Medication Reviews are completed for the next year and beyond. The MCC, Angie Monk, will follow up with the Residential Advisors (RA's) to ensure apppointments are completed within the required peroids, i.e. within 90 days. The MCC, Angie Monk sent an email to all of the Emmaus Community RA's on 9/4/18, requesting information regarding 163 (c), dates of recent appointments, etc. Through an internal review the only individual found to be out of compliance with 163 (c), at 100 days was individual #1 from this 2018 State Inspection. Merck Clinic was contacted on 8/28/18. The Residential Advisor, Sandy Cerminara and Allen Gregory, Director (contacted also on 9/4/18), thought that a letter had been sent to ensure her medication review. However Allen Gregory was on vacation, as well as RN, Cynthia Small during Emmaus' inspection on 8/28 and 8/29/18. Individual #1 was seen however on 7/20/18 and is within her Psychiatric Physician's care. Individual #1 should have her next Psychiatric Medication review/or appointment with her Psychiatrist by 10/18/18, in accordance with 163 (g) regulation. Angela Monk, MCC will continue follow up and follow through with the assistance of the Residential Advisor, Sandy Cerminara for completion of the next Psychiatric appointment of individual #1. 09/08/2018 Implemented

Emmaus has changed their Physician's Medical Form as of 09/27/2016 to include a line that states, "Please list Medical information pertinent to diagnosis and treatment in case of an emergency:" A line is also provided for comment by the Physician. This statement is on page 2 of 2 of the document. However, for the record, Emmaus feels that our form has been utilized since 2011 and previous BHSL Inspectors did not comment on this absence. The entire form encompasses medical information that is pertinent to diagnosis and treatement in case of an emergency. This new Physician's Medical Form will be utilized throughout the agency for all of our individuals under the 6400 Regulations. [Individual #1 (AS 11/1/16)] will take this new form with her to her Annual Physical exam in April of 2017. [Individual #1's (AS 11/1/16)]Residential Advisor, Sara Harris, will ensure that the new updated Physician's Medical Form, revised 09/27/2016, will be used.[Individual #1's medical information will be updated to included medical information pertinent to diagnosis and treatment in case of an emergency. Immediately and upon completion, the Director of IDD services shall review all individuals' physical examinations to ensure all required information is included and there are not areas of required information left blank.
