This review will verify that the physical form includes specific precautions that must be taken if the individual has a communicable disease to prevent spread of the disease to other individuals. If any required sections are incomplete, DSP staff will immediately request that the attending provider complete the necessary information before the form is finalized, effective immediately. Any issues with obtaining the requested documentation will be reported immediately to the House Manager and Program Specialist. Additionally, the QCH House Manager has been retrained ( 10/13/2025) on the critical importance of ensuring all physical examination forms are accurate and complete before submission. This retraining emphasized the necessity of proper documentation to support regulatory compliance and continuity of care. |
