6400.68(b)Hot water temperature was measured at 129.2 degrees Fahrenheit in second floor main bathroom sink and throughout home during the physical site inspection at approximately 9:30am Hot water temperatures in bathtubs and showers may not exceed 120°F. During the physical site inspection of the Emmett Street home, the hot water was tested and it read 129.2 degrees Fahrenheit. On 6/3/19 Residential Manager DG reached out to all of the house staff to ask if someone had possibly adjusted it, as during his monthly water check (which was completed on 5/20/19) it had read 119 degrees Fahrenheit. One of the house staff replied that she had turned the hot water up in order to sanitize bedding, as she felt 120 degrees Fahrenheit would not be hot enough to sanitize; however forgot to switch it back. Residential Manager DG put in a maintenance request, and the water was adjusted on 6/3/19 and tested on 6/4/19 and the results were exactly 120 degrees Fahrenheit. (Attachment will be sent of picture of thermometer w/ reading) Additionally the Program Specialist (MH) will do re-training on 6/26/19 for all staff who work residentially. This training will include the importance of never adjusting water temperature (as the regulation states no higher than 120 degrees Fahrenheit), the dangers of water being set higher than 120 degrees Fahrenheit (i.e. causing burns, especially to those who do not have the skills present to regulate water temperature independently), and lastly to inform staff that if something needs to be washed in water hotter than 120 degrees Fahrenheit, they may access a laundromat to do so. 06/26/2019 Implemented
6400.110(e)The Fire alarm system was not interconnected throughout the three levels of the home. The Nest system was not connected to the internet at the time of the fire alarm testing. If the home serves four or more individuals or if the home has three or more stories including the basement and attic, there shall be at least one smoke detector on each floor interconnected and audible throughout the home or an automatic fire alarm system that is audible throughout the home. The requirement for homes with three or more stories does not apply to homes licensed in accordance with this chapter prior to November 8, 1991. At the time the fire alarm was tested at Emmett Street location on 5/30/19, the Nest system was not properly connected; causing the smoke detectors to not be audible at the same time throughout the home. Residential Manager DG contacted the Nest support team on 6/4/19 and was able to resolve the issue by purchasing a Wi-Fi extender, and resetting the system. All smoke detectors are now interconnected/audible throughout the home. Additionally, Residential Manager DG downloaded the Nest app onto his cell phone, which will notify him immediately if for any reason the system is not working properly (i.e. back-up batteries are dead, system loses Wi-Fi connection, etc.) so that he is able to contact the Nest support team right away to have it re-connected or if needed (the system cannot be fixed at that time) make plans to relocate the ladies that live In that home. 06/04/2019 Implemented
