6400.77(b)On 6/11/25, the first aid kit in the home did not contain a thermometer. A first aid kit shall contain antiseptic, an assortment of adhesive bandages, sterile gauze pads, a thermometer, tweezers, tape, scissors and syrup of Ipecac, if an individual 4 years of age or younger, or an individual likely to ingest poisons, is served. 6400.82(f)On 6/11/25 at 11:05AM, the second-floor bathroom did not contain soap, toilet paper, individual clean paper or cloth towels, or a trash receptacle.Each bathroom and toilet area that is used shall have a sink, wall mirror, soap, toilet paper, individual clean paper or cloth towels and trash receptacle. Trash can had been removed from the bathroom by Individual and taken to the downstairs bathroom. Immediately upon discovery, trash can was returned to the upstairs bathroom on 6/11/2025. A cloth hand towel has been placed in the upstairs bathroom instead of disposable paper towels due to Individual's propensity for clogging the toilet with toilet paper and paper towels. A toilet paper dispenser has been installed on the wall immediately outside the bathroom as of 6/26/2025 where Individual still has access to it, but where Individual will likely be less inclined to unroll the toilet paper and stuff into the toilet. Behavior Support Plan has been updated as of 6/19/2025 to address the behavior of clogging the toilet. 07/31/2025 Implemented

In the general health and safety risks section of Individual #1's individual plan, last updated on 6/4/25 reads, "[Individual #1] will avoid potential sources of heat but may not avoid them until after he has caused himself some harm." Individual #1's initial assessment, completed 2/7/25 indicates Individual #1 can control the water temperature, understanding the difference between hot and cold and adjusting the temperature to be appropriate. In the water safety section of Individual #1's individual plan, last updated 6/4/25 reads, "[Individual #1] loves bathing in hot water, he is not able to temper water temperature without assistance."The individual plan shall be initially developed, revised annually and revised when an individual's needs change based upon a current assessment.Residential program specialist emailed SC on June 18th, 2025 requesting Individual #1's ISP be updated to match current annual assessment which states that Individual #1 can temp the water to his desired temperature without assistance. 07/03/2025 Implemented
