The pharmacy issued medication label was observed with an origination date of 4/10/2025 and "DO NOT GIVE" written over the label in black permanent marker. Individual #1's Clotrimazole 1% Cream should have been discontinued and disposed of on or around 4/25/2025; however, on 2/11/2026 at 11:50 AM, the medication was still located on-site in the residential home.Prescription medications that are discontinued or expired shall be destroyed in a safe manner according to Federal and State statutes and regulations.The medication was removed from the site on 2/11/26 by the Program Specialist. The Agency's current procedure related to Regulation 6400.163h states: The procedure for removing and disposing of medications is to remove them from the Community Home and secure them in the hazard container in the Corporate Office immediately. The Residential Program Director/Training Coordinator coordinates with the pharmacy for disposal of all medications. Effective 4/1/26 the Agency's revised procedure is: the Med Support Staff will be responsible for removing discontinued, expired, and/or time limited medications from the Community Home within 24 hours of the medication change being implemented.
