6400.188(d)Direct Service Worker #1 kept Individual #1's personal items including eye shadow, lip stick and nail polish locked. Individual #1, date of birth 10/25/59 had to request his/her personal items from Direct Service Worker #1. Direct Service Worker #1 would tell Individual #1 that s/he was not allowed to have drinks in the living room of the home. In addition, Direct Service Worker #1 would tell Individual #1 not to wear socks with his/her slippers. Individual #1 expressed disgust with being told what to do by Direct Service Worker #1. The residential home shall provide services that are age and functionally appropriate to the individual.After completing our investigation in October the Agency redesigned it Positive Approaches training and retrained all staff during October and November. DSW # 1 was retrained and then reassigned to work at the Adult Training Facility where he/she works under close supervision. In January of 2017 staff discussed Dignity and Respect at their monthly staff meetings and a poster was placed at all of the sites. This morning, March 13, 2017, program specialists and supervisors were trained on 188(d). They will be training all of the direct care staff during the months of March and April, with training to be completed by April 30th 2017. This training will be added to positive approaches for future training and new hires. I will forward copies of the the Positive Approaches training power point, DSW #1's training summary, the Dignity and Respect posting, and the newest acknowledgement form and training for Age and Functionally Appropriate Services via email. It is the responsibility of Program Specialists and Supervisors to monitor direct care staff closely and assure that they are providing services that are appropriate at all times. [At least monthly for 6 months and continuing at least quarterly thereafter, the program specialist(s) shall meet privately with each individual to give the individual the opportunity to discuss any concerns with regards to staff treatment to ensure all individual are provided services that are age and functionally appropriate and treated with dignity and respect including being treated age and functionally appropriate. 6400.141(c)(7)The most recent gynecological examinations including a breast examination and a Pap test, for Individual #1 date of birth 6/17/89, were completed on 11/5/14 and 11/25/15.The physical examination shall include: A gynecological examination including a breast examination and a Pap test for women 18 years of age or older, unless there is documentation from a licensed physician recommending no or less frequent gynecological examinations. Medical appointments for the individuals are scheduled and completed by Residential Program Supervisors and in some cases one on one staff. All of these staff have an annual calendar of appointments for the individuals they serve. These staff will be retrained on the requirements for completing annual appointments (including 141(C) (7)) before May 16th 2016. They will review their individual records to assure that all annual medical reports have been completed in a timely manner by May 26th 2016.They will review their annual calendars with the individual's program specialist as well. All completed medical reports, including annuals, will be given to the program specialist for them to review before they are scanned and filed. If for any reason a practitioner cancels or reschedules an annual appointment the RPS or 1:1 staff will notify the program specialist immediately. The Program Directors will spot check three records per month to assure that this procedure continues and that all annual appointments are being completed in a timely fashion.
