| 6400.151(b) | Staff #1-physical exam dated 11/8/18, did not include the date signed by the physician. | The physical examination shall be completed, signed and dated by a licensed physician, certified nurse practitioner or licensed physician's assistant. | Staff #1 had a physical examination completed and signed by the physician however the physician failed to date next to his signature. To correct this, Staff#1 had a physical completed on 4/8/2019 that included the physician's signature and date that the examination was completed. Going forward the employee physical examination must be completed, signed and dated by a licensed physician, certified nurse practitioner or licensed physician assistant prior to employment at Comhar. The recruiter and credentialing specialist will review physical forms for completeness including exam date signed by the licensed physician. Employees will not start employment with COMHAR until this information is received and reviewed. Additionally, this process will be implemented for tenured staff every two years when it is time for the re-examination. |
