6400.67(a)Surfaces shall be in good repair. At the time of the inspection the wall which borders the shower in the main bathroom had what appeared to be water damage. This damage was causing the wall to be soft to the touch and will need to be repaired. The bathroom located in the individual's bedroom also had similar type of water damage to the wall bordering the shower as well.Floors, walls, ceilings and other surfaces shall be in good repair. Agency Residential Director, Frank Kibaara, coordinated with the Maintenance staff to repair the walls near each respective shower in both bathrooms. A first aid kit shall contain antiseptic, an assortment of adhesive bandages, sterile gauze pads, a thermometer, tweezers, tape, scissors and syrup of Ipecac, if an individual 4 years of age or younger, or an individual likely to ingest poisons, is served. Residential Director purchased scissors and placed them in the First Aid Kit. One of the individual's who resides in the home took the scissors and would not return them. 04/13/2022 Implemented
6400.32(r)(1)Individual #4 and Individual #5's bedroom doors had a lock with a coin key lock. The individuals and staff did not have a coin to access this lock. Individual #5 expressed that the individual would prefer a lock with a key on the door.Locking may be provided by a key, access card, keypad code or other entry mechanism accessible to the individual to permit the individual to lock and unlock the door.Maintenance Staff replaced the Bedroom door mechanisms with the proper locking and key devices for the Individual's use. These keys have been provided to the individuals and staff. 04/15/2022 Implemented
