| 6400.165(g) | Individual #1 takes psychotropic medications and sees a psychiatrist for medication reviews, which are documented on "Medication Review" forms found within the Individual Record. The Medication Review forms for the following dates were missing documentation of the need to continue each medication: 08/14/2024, 06/26/2024, 06/12/2024, 05/09/2024, 03/28/2024, 03/21/2024, and 03/14/2024. | If a medication is prescribed to treat symptoms of a psychiatric illness, there shall be a review by a licensed physician at least every 3 months that includes to document the reason for prescribing the medication, the need to continue the medication and the necessary dosage. | Staff member took the individual for a counseling appointment and took the wrong form them. Program staff members has been retrained on the various agency appointment forms for our individuals. |
