During the ODP site inspection on December 18, 2025, a violation was issues because no documentation was available to show that Individual #5 had an exam completed. This individual date of birth is 7/3/2005, which makes her 20 years old and of age to be recommended for this exam. On 1/8/2026, the supervisor spoke with Individual #5 again to encourage her to participate in this required test and to share with her the importance of these tests for women but the individual once again refused it. |
