| This individual's maternal aunt and uncle were named as emergency contacts. We only had their full names, home phone numbers, and cell phone numbers. This violation happened because we did not have their address listed and did not realize that was missing. This regulation is important because if someone is listed as an emergency contact, we should have all possible contact information for them in the event of an emergency.
This was discovered during inspection on 7/21/26. The team leader of this home reached out to the individual's mother and got the address for her aunt and uncle (married). This was updated in her online chart this same day, and new face sheets were printed 7/24/26.
