6400.67(b) | The hose leading from the canister to the nozzle on the fire extinguisher located in the basement was covered with a substance resembling mold. The substance had a bloom-like appearance and a fuzzy surface consistent with mold. There appeared to be of a problem of excess moisture in the basement which could lead to the growth of molds and mildew and could be a hazard to the health of the individuals and staff in the home. The basement felt and smelled damp even though a dehumidifier was in operation at the time. | Floors, walls, ceilings and other surfaces shall be free of hazards. | 10/8/2024 Facilities Manager inspected the basement and found the dehumidifier to be working incorrectly. New dehumidifier purchased and installed on 10/9/2024. All locations were inspected and updated dehumidifiers were installed at two additional locations. Walk through of basement completed and substance that resembled mode on hose was cleaned. |

6400.163(d) | Individual #1 is not able to self-administer medication and their medication is stored in a locked area. Individuals #2, #3 and #4 are able to self-administer their medication. At the time of the inspection, the medications for Individuals #2, #3 and #4 were stored in separate, non-locking containers, one container for each individual, in an unlocked cabinet in the dining room of the home that is accessible to all of the individuals in the home. Medication must be stored in a locked area or, if able to self-administer, in an area that is not accessible to others such as the individual's bedroom, or in a way that allows the individual who self-administers to access their own medication but prevents other individuals from accessing the medication. | Prescription medications and syringes, with the exception of epinephrine and epinephrine auto-injectors, shall be kept in an area or container that is locked. | Review and updated policy regarding storage of medication for Individuals who self administer medication. Lock boxes were ordered on 10/9/2024. Individuals 2, 3, and 4 medications were relocated to Individual lock box's with combinations for each Individual to lock and unlock their own medication box. Individuals 2, 3, & 4 meds were locked in cabinet on 10/7/2024 until boxes arrived and were put into use on 10/14/2024 |
