There is a skylight in this bathroom, but at the time of inspection there was no screen in this skylight. On 5/22/26 a small fan was place in the bathroom for ventilation. Additionally, an order for the replacement screen for the skylight was placed on 6/10/26. This home has a telephone on the first floor and the basement. The individual that lives in this home used the phone in the basement to speak to family and did not place the basement phone on the charger. The phone on the main floor was operable, and this was demonstrated at the time of inspection. The basement phone was immediately placed on the charger and was fully charged by that evening. 05/21/2026 Implemented

6400.68(b)The water temperature read 140 degrees Fahrenheit during the time of inspection. The water temperature was checked the following day and read 114 degrees Fahrenheit, so water was adjusted properly. Hot water temperatures in bathtubs and showers may not exceed 120°F. Keystone regularly checks the water temperature in each home to ensure that it is within regulatory guidelines. When the water temperature measured this high during inspection, we looked into how this could be. We discovered that a particular staff was raising the water temperature on the hot water heater on the overnight to get the water hotter for cleaning and sanitation and on this day, she forgot to lower it. This staff person was counseled on the greater risk of raising the temperature and to not do this again. Keystone lowered the water temperature and it measured the correct temperature the evening of 6/27/22. The following day, the Lead Inspector revisited the home to retest the water to ensure that the temperature was within the guidelines; it was satisfactory 06/28/2022 Implemented
6400.73(a)A handrail is needed for the basement stairs. There is no railing for the bottom 6 stairs in the basement. Each ramp, and interior stairway and outside steps exceeding two steps shall have a well-secured handrail. This home was newly acquired and licensed this year. There was a handrail on the bottom portion of the steps but there wasn¿t one on the top portion of the steps. Our maintenance department installed a handrail on 7/20/22. 07/21/2022 Implemented
6400.186Individual #3's assessment states that all poisons are kept locked, however during the time of inspection there were cleaners unlocked In a basement closet in which the individual has full access. The assessment needs to match what is being implemented in the home.The home shall implement the individual plan, including revisions.Staff had been cleaning and doing laundry that morning and failed to secure the cleaning supplies once they were done. Individual #3 is assisted with one to one staffing and is always in the presence of staff when in the basement or when using cleaning supplies. The cleaning supplies were immediately locked up and secured and counseled on the necessity of keeping these items secure. 06/29/2022 Implemented
