Fully Implemented - PE - 2-14-13 | (b) Floors, walls, ceilings and other surfaces shall be free of hazards. | The Supervisor, with the assistance of maintenance, has been made aware of all sockets located within a site and thus all supervisors and staff will insure that all light bulbs are working and all light sockets contain light bulbs. Staff will replace bulbs as needed and contact maintenance if assistance is needed.
A work order tracking form was developed covering all homes with staff inservice of proper notification when repairs are necessary. |

6400.112(d) | A fire drill dated October 22, 2011, had an evacuation time of 2 minutes and 33 seconds. Fully Implemented - PE - 2-14-13 | (d) Individuals shall be able to evacuate the entire building, or to a fire safe area designated in writing within the past year by a fire safety expert, within 2 1/2 minutes or within the period of time specified in writing within the past year by a fire safety expert. The fire safety expert may not be an employee of the home or agency. Staff assistance shall be provided to an individual only if staff persons are always present at the home while the individual is at the home.
| All staff at the site have been retrained on 12/6/12 in the proper procedure for running and documenting a fire drill. This was done by Director of Residential. All DSS Supervisors have been retrained in the proper procedure to run and document a fire drill and insuring that documentation is thorough and correct. DSS Supervisors are responsible for reporting any discrepancies in fire drills to Program Specialists and Residential Director as well as reporting any issues with the integrated fire system or smoke detectors not working correctly. It has also been emphasized by all staff that they are to be cautious and accurate in filling out each specific area of the fire drill report to insure that the information is accurate and complete.
