Communication was made to staff to be sure the individuals have a mirror in their room at all times. If they discover there is no mirror, they are to report it to management immediately. Management has stressed the importance of each item and will have the Compliance Officer do checks to oversee the House Manager. This writer will email a picture of individual 2's current bedroom. A mirror was hung the day of inspection to satisfy regulation. |

Vision and hearing screenings were not completed until the next physical examination on 9-1-22. | The physical examination shall include: Vision and hearing screening for individuals 18 years of age or older, as recommended by the physician. | LPN has been retrained on the importance of reviewing the completed physical form and understanding what the physician has written.
