| 6400.82(f) | At the time of inspection, there was no soap in the home's bathroom. | Each bathroom and toilet area that is used shall have a sink, wall mirror, soap, toilet paper, individual clean paper or cloth towels and trash receptacle. | According to the individual's ISP, he is not safe around poisonous materials. UCCH kept the hand soap in a locked closet across the hall from the bathroom and would assist the individual with washing his hands and relocking the soap for his safety. UCCH was not aware of other safe soap options. Below is the statement from his ISP.
Safety Precaution: Jeffrey does not recognize danger of any kind. He is unfamiliar with poisonous substances and cleaning products. Poisons are out of reach at home. He may not recognize the difference between edible and inedible substances.
The program manager got a non-toxic soap immediately after the inspection on 1/22/25 from the stock in the office, and placed it in the individual's bathroom and made accessible to the individual in his home. |

The corresponding area of the form was left blank. | The physical examination shall include: A signed statement that the staff person is free of communicable diseases or that the staff person has a communicable disease but is able to work in the home if specific precautions are taken that will prevent the spread of the disease to individuals. | Human resources manager stated she checked only for the TB result, physician's signature (which covers the rest of the questions asked on the form including free from communicable disease), and clinic stamp. Once the omission was noted on 1/22/2025, the physical examination form was sent back to the physician immediately to complete the missing section.
