Two cans were against the garage, close to the front of the home. One can was against the shed, in the backyard.Trash outside the home shall be kept in closed receptacles that prevent the penetration of insects and rodents.One of the three garbage cans was not an officially accepted bin (by the garbage company) and it has been removed from the property, The House Manager will be retrained by the Program Specialist on making sure to check the status of the garbage throughout the week. If at any time the garbage can is overflowing (or near overflowing) the House Manager will contact the Maintenance department to have the excess garbage removed. 05/24/2021 Implemented
6400.181(e)(7)Individual #1's 8/1/2020 assessment states "uses lighter safety-N (according to the key for the assessment "N" means no). At 10:45 AM, 4/22/21 a red, BIC lighter was located unattended, on the back porch, on a black lawn chair.The assessment must include the following information: The individual's knowledge of the danger of heat sources and ability to sense and move away quickly from heat sources which exceed 120° F and are not insulated. 6400.32(d)The Office of Developmental Programs requires that staff who provide direct services wear a mask that covers the nose and mouth during the entirety of service provision. Direct Services Workers #1 and #2 at the home were not wearing masks upon arrival at 10:04 am on April 22, 2021. Direct Service Worker #3 at the home was not wearing a mask at 10:11 AM on April 22, 2021. Failure to wear masks is undignified and disrespectful in that it creates a risk of transmitting the COVID-19 virus from staff to individuals.An individual shall be treated with dignity and respect.The Program Specialist did a training with all of the house staff members at the home.
