6400.166(b) | Individual number one is prescribed Gunafacine 1mg tab (take ½ tablet by mouth 3 times a day 8am, 4pm, 8pm). On 8/8/24 at 4pm, staff initials (#1) appear on the MAR indicating that the medication was administered at that time. However, the medication remains in the blister pack for that administration time. On that date, the individual was hospitalized. In discussion with the staff, they were aware that the medication at that time was not administered, and recognized this was a documentation error and it should have been appropriately documented as such and indicated on the back of the MAR. | The information in subsection (a)(12) and (13) shall be recorded in the medication record at the time the medication is administered. | The odp medication certified trainer retrained direct support professional , in discussion with staff we were made aware that the medication at the time was not administered and recognized this was a documentation error and it should have been appropriately documented as such and indicated on the back of the MAR |
