The outside of the building and the yard or grounds shall be well maintained, in good repair and free from unsafe conditions.The moss and leaves debris was removed from the side exterior stairs immediately after the exit interview the day of inspection, 9/25/25. 6400.81(i)On 9/25/2025 at 12:05PM, there were no curtains or blinds covering the window to the right of the bed in Individual #1's bedroom.Bedroom windows shall have drapes, curtains, shades, blinds or shutters. At the time of inspection, the curtain had been removed for cleaning due to visible hand stains although previously washed the prior weekend. The curtain was in the washer and then put in the dryer during the inspection, as indicated to the inspector. The curtain, which is hung using a tension rod, was reinstalled upon completion of drying. Both the rod and curtain were promptly returned to the window the same day, restoring full compliance. 09/25/2025 Implemented
6400.101On 9/25/2025 at 12:40PM, a turn lock and a deadbolt locking mechanisms were on the basement side and a keyed locking mechanism on the garage side of the door, between the basement and the garage. These locking mechanisms pose an obstructed egress from the garage when engaged. There is no swing door in the garage.Stairways, halls, doorways, passageways and exits from rooms and from the building shall be unobstructed. The locking hardware on the door between the basement and garage has been replaced with a non-keyed, single-action latch that allows for unobstructed egress from both sides. 09/29/2025 Implemented
