6400.181(a) | Individual #1, date of admission 12/05/23, had an initial assessment completed on 03/05/24. This exceeds the 60 calendar days after admission. | Each individual shall have an initial assessment within 1 year prior to or 60 calendar days after admission to the residential home and an updated assessment annually thereafter. The initial assessment must include an assessment of adaptive behavior and level of skills completed within 6 months prior to admission to the residential home. | Given that the assessment was completed but outside of the statutory timeline, OAHC emphasized ensuring that this was correct for future assessments. Accordingly, OAHC's residential management team, including the president, residential facilities director, and program specialist met to retrain on OAHC's programming guidelines for accuracy, clarity, and competency. It was reviewed that the initial residential assessment must be completed within 60 days of admission. All three parties agreed to fulfill this expectation if needed and the program specialist will be responsible on a routine basis. OAHC also modified its residential assessment to include a clarification that the initial assessment date must be within 60 days of admission. |
