6400.141(a) | An individual shall have a physical upon admission and annually thereafter. Individual #1 had an annual physical dated 6/21/24, however it was missing several regulatory components required on the physical. Due to the amount of missing sections, it is being cited as the physical being incomplete. | An individual shall have a physical examination within 12 months prior to admission and annually thereafter. | The agency reached out to the family and the Medical Provider for the full Progress notes for the visit that accompany the Agency Physical Form. The family historically does not allow the agency staff to accompany or make appointments for their family member who we serve. We received the Progress notes and will send for review and acceptance. We also have edited the Agency Physical form for clarity and full documentation to each requirement. This form will not only be sent to the family prior to the appointment but Faxed to the Medical Provider prior to the appointment as well. Please see document images sending via email. |

6400.181(f) | The assessment is dated 2/12/25 , there is a letter that was sent to "family members and designated persons" however there is no date on the letter to distinguish when the letter was sent and if it was indeed 30 days prior to the ISP mtg. | The program specialist shall provide the assessment to the individual plan team members at least 30 calendar days prior to an individual plan meeting. | The agency Program Specialist, Beth Sell, has added the date to the Letter at the time of the Inspection. All Agency Assessment Transmittal Letters will be fully dated at the time of inception and held in the record moving forward to comply with the requirement for assessment to plan team members at least 30 calendar days prior to an Individual's Planning Meeting.
