The outside of the building and the yard or grounds shall be well maintained, in good repair and free from unsafe conditions.In order to correct this violation, the three-foot by four-foot plywood board covering the drying concrete pad was immediately removed by the maintenance technician, who explained to the surveyor that he had placed it over the concrete to aid in dying. Upon learning that this could result to be cited, he promptly removed the board during the inspection to eliminate any tripping hazard. 6400.62(a)At 10:45AM, a spray bottle of Clorox Clean-Up, an aerosol can of Easy-Off Oven Cleaner and Lysol Disinfectant Wipes were unlocked and accessible in the cabinet under the sink in the kitchen of the home. At 10:56AM, a 32-ounce spray bottle of Windex was unlocked and accessible on the floor in Individual #1's bedroom. At 11:16AM, a bottle of Goof Off Hand Cleaner and a container of All Might Pacs laundry pods were unlocked and accessible in the front room of the basement. Individual #1's assessment, completed 8/15/2024, states that the individual requires verbal prompts to use poisons and physical prompts to avoid poisons. [Repeat Violation, 3/7/2024]Poisonous materials shall be kept locked or made inaccessible to individuals. In order to correct this violation, immediately a file review was conducted. 6400.64(f)At 10:25AM, a cracked trash receptacle containing discarded items and the lid of the trash receptacle was on the deck in the back of the home.Trash outside the home shall be kept in closed receptacles that prevent the penetration of insects and rodents.To correct this violation, the trash can in question used by the maintenance worker to collect debris around the home and in the basement during the unannounced inspection that day was discarded and put out for waste management pick up immediately by the maintenance worker. At 10:55AM, a mirror and piece of wood that appears to be a detached headboard were unsecured and leaning against the wall adjacent to the air conditioner in Individual #1's bedroom. 6400.72(a)At 10:24AM, the unscreened window in the dining room facing the back deck of the home was open at the top and could not be closed leaving space for insects and rodents to enter the home. [Repeat Violation, 3/7/2024]Windows, including windows in doors, shall be securely screened when windows or doors are open. To correct this violation, the window was boarded up immediately by maintenance staff. The window in question, was busted out by an individual. The agency was working toward replacing the window prior to this unannounced inspection. A second estimate and measurements were taken on August 1, 2024, by a window replacement company. The entire window frame will be replaced on September 23, 2024 with a new screen. 6400.80(a)At 10:16AM, the concrete walkway leading to the front steps had a crack measuring approximately ten inches on the right side of the walkway at the bottom of the steps. The pad was loose and moved under foot when walked on, creating a potential tripping hazard. At 10:21AM, multiple pieces of wood with nails protruding upward were on the ground on the side of the home leading to the backyard and under the deck in the back of the home. Outside walkways shall be free from ice, snow, obstructions and other hazards. To correct this violation, the walkway leading to the front step was leveled with cement and the ramp/¿ pieces of wood with nails protruding upward¿ that the handicap neighbor used for her electric wheelchair was removed from a shared driveway leading to the rear of multiple rowhouses was removed immediately by maintenance staff. 09/30/2024 Implemented
6400.80(b)At 10:07AM, a broken gate and chain link fencing were leaning against a bush to the right of the walkway leading to the front porch of the home. A 10:22AM, a pipe measuring approximately eighteen inches high by four inches in diameter was protruding from the walkway at the rear of the home. The pipe did not have a lid and was not protected from insects and rodents. At 11:40AM, screws were protruding from the underside of the wooden deck in the back of the home posing laceration and puncture hazards. At 11:41AM, two large extension ladders were leaning against the chain link fence in the backyard of the home posing a crushing hazard. [Repeat Violation, 3/7/2024] The outside of the building and the yard or grounds shall be well maintained, in good repair and free from unsafe conditions.To correct this violation the following tasks were performed immediately by maintenance staff; the gate was removed from the walkway leading to the front porch, the protruding nails were removed from under the deck, and a cap was placed over the downspout drain. 09/30/2024 Not Implemented
