The wall bordered the exterior of the home, which was also holding the air conditioning unit for the home. At the time of inspection, the wall in this area was soft to the touch from what appeared to be water damage.Floors, walls, ceilings and other surfaces shall be in good repair. Provider agency contacted landlord/property owner to address repair needed to wall on the following date: 7/9/2024. After the landlord/property owner was unresponsive to requests made for repairs, provider agency hired a contractor to complete required repairs, which occurred on 7/19/2024. Due to weather the occurred during the weekend of the inspection, the wall remained damp/moist. The contractor returned for additional repairs, completed 7/27/2024. 09/01/2024 Implemented

6400.101The exit door from the garage to the exterior of the home has a key lock that requires a key to be utilized to unlock the door to exit the home. In the event of an emergency, individuals and staff would not be able to utilize that exit unless they had a key on their person, was able to utilize a key and would impede the ability to evacuate quickly from the home.Stairways, halls, doorways, passageways and exits from rooms and from the building shall be unobstructed. Provider removed the key lock that required a key to be utilized to unlock the door to exit the home. The old key lock was replaced with a new deadbolt which does not require a key to be utilized to unlock the door. 04/03/2023 Implemented
