Responsible Person: The Maintenance Supervisor will coordinate with Expert Glass Company to replace the screen. Timeline for Change: Expert Glass Company will come to the location, inspect the work and let Avid Care know when they are able to come back and replace the screen. Avid Care will ensure that the screen is replaced no more than 30 days from 7/28/24. To address the issue immediately, Avid Care will place a patch over the torn screen to prevent any insects from coming into the home. Method of Implementation: Once the screen is replaced, Avid Care will ensure the window is in good repair prior to Expert Glass Company leaving the premises. |

| 6400.101 | At 12:30PM, there was a turn lock on the hallway side of the door between the hallway on the first floor and the basement of the home posing an obstructed egress from the basement when engaged. At 12:32PM there was a keyed lock on the basement side of the door between the basement and the garage of the home posing an obstructed egress from the garage when engaged. There is not a swing door in the garage. | Stairways, halls, doorways, passageways and exits from rooms and from the building shall be unobstructed.
| Changes to be made: The turn lock on the basement exit door will be removed to ensure the door does not posed an obstructed egress, in compliance with Pennsylvania Code § 6400.101.Responsible Person: The Maintenance Supervisor will be responsible for removing the turn lock. Timeline for Change: The turn lock will be removed by 8/1/24. Method of Implementation: The Maintenance Supervisor will remove the turn lock from the basement exit door and verify that the door is fully functional and unobstructed after the removal. |
