The receptacle was being used for cigarette butts and was cracked and broken around the top lip of the receptacle. The receptacle was filled to the top with cigarette butts, and there were dozens of cigarette butts strewn about on the ground. The outside of the building and the yard or grounds shall be well maintained, in good repair and free from unsafe conditions.Supervisor did not ensure the ashtray was empty into the trash bin. 6400.186The home shall implement the Individual plan. The current approved Individual Support Plans (ISP) for both individuals residing in the home state that the individuals are safe with poisons and poisons are not locked in the home. The current approved ISP last updated on 12/01/2025 for Individual #1 states that the individual can safely use and/or avoid poisons and that poisons are not locked in the home. The current approved ISP last updated on 12/02/2025 for Individual #2 states that they are safe around poison/cleaning products and that they do not need to be locked in the home. At the time of the inspection, household cleaning products were found stored in a locked closet near the kitchen. Individual #1 had a Restrictive Procedure Plan for the locking of knives and sharps in the recent past. The restrictive procedure has since been faded, and is no longer in effect, per the Behavior Support Specialist who wrote the plan. At the time of the inspection, knives and scissors were found stored in a locked closet near the kitchen.The home shall implement the individual plan, including revisions.The poison should not have been locked; the individuals were relocated at the home recently the supervisor forgot to unlock the poison.
