6400.141(c)(10) | The physical examination for Individual #1, dated 3/9/15, did not indicate whether the individual is free of communicable disease; therefore compliance could not be measured. | The physical examination shall include: Specific precautions that must be taken if the individual has a communicable disease, to prevent spread of the disease to other individuals. | The physical exam form will be resubmitted to the Primary Care Physician by the House Supervisor in order for the physician to verify Individual #1 is free from communicable diseases by 6/1/2016. A written process for medical follow-up, annual appointments, completion of forms and compliance with required dates will be established by 6/1/2016. Staff will be given the written process. Program Directors will ensure the process is being followed. [Individual #1's medical information was updated by PA-C, medical professional on 3/30/16 to include that Individual #1 is free of communicable disease and has a negative PPD on 3/1/15. Immediately, the program specialist or designated staff person will review all individuals' current physical examination to ensure all required information is present and will obtain missing information. Prior to entering into the individuals' records the program directors or program specialists will review all individuals' physical examinations to ensure required information is documented and will obtain missing information.
