6400.101On 3/5/26, at 11:55 AM, the door leading to the attic in the home had a locking mechanism on the door with a key required on both sides of the door, preventing egress from the attic if the key was not present..At 12:03 PM Individual #1's bedroom door lock was a skeleton key lock and if engaged the individual would not be able to exit the room without a key.Stairways, halls, doorways, passageways and exits from rooms and from the building shall be unobstructed. The Facility Manager disabled the locking mechanisms of the skeleton key locks on both the attic door and the individuals bedroom at this location. The Facility Manager then added a dead bolt lock to both doors above the door handle. This allows both locations to be locked with a key from the outside for privacy requirements, but allows any occupant on the other side to unlock the door via a thumb latch so that a key is not required for egress from any room.
