Individual #1's individual plan, last updated 5/20/2026 reads, "[Individual #1] requires verbal prompts and supervision to evacuate safely during fire drills at school and in the event of a fire at home." | The home shall notify the local fire department in writing of the address of the home and the exact location of the bedrooms of individuals who need assistance evacuating in the event of an actual fire. The notification shall be kept current.
| The home will submit an updated written notification to the local fire department that includes the home's address and the exact bedroom location of Individual #1, who requires verbal prompting and supervision to evacuate safely. A copy of the updated, dated notification (and proof of delivery/receipt) will be retained in the home's records. The Program Specialist will review all current individuals' assessments and individual plans against the existing fire department notification and correct any other discrepancies identified. |
