| 6400.151(a) | Staff #1 had physicals completed on 2/20/23 and 3/6/25. The physicals were not completed within the two-year time frame as required. | A staff person who comes into direct contact with the individuals or who prepares or serves food, for more than 5 days in a 6-month period, including temporary, substitute and volunteer staff, shall have a physical examination within 12 months prior to employment and every 2 years thereafter. | Staff #1 had physical examinations completed on 2/20/23 and 3/6/25, which exceeds the required two-year timeframe. During review, it was identified that Human Resources (HR) believed Staff #1 did not need to complete a physical because she was pregnant at the time the physical was due. This assumption was incorrect, and pregnancy does not exempt staff from meeting mandatory physical requirements.
The Program Director's audit identified this issue and immediately instructed HR that all staff, regardless of pregnancy status or other circumstances, must complete physicals within the required two-year timeframe. HR has been retrained on the policy and informed that no staff condition may delay or exempt completion of required health evaluations. A physical was completed for Staff #1 on 3/6/25 |
