6400.80(b) | There was assorted debris such as an old pair of gloves, old watering can, hand towel, broken flowerpot, etc., laying in the side yard that needs to be cleaned up in order to keep the outdoor area well maintained. | The outside of the building and the yard or grounds shall be well maintained, in good repair and free from unsafe conditions. | The debris was removed from the yard by HAP inc, program director on 6/28/24. The yard is now free from debris (Attachment 12 and 17). Maintenance Director checked all of the other homes for debris as part of his regular weekly rounds 7/1/24 through 7/6/24. No other debris was found at any of the homes. |

6400.111(f) | The tags on the fire extinguishers located in the garage and in the basement, both indicated that they were last inspected in January 2023 and are overdue to be inspected and approved annually by a fire safety expert. | A fire extinguisher shall be inspected and approved annually by a fire safety expert. The date of the inspection shall be on the extinguisher. | Both of the extinguishers with the inspection date of January 2023, were also tagged 2024 and had in fact been inspected within the appropriate time frame based on records reviewed by John S. Posen Inc. Representative from John S. Posen Inc. corrected the incorrect date on both the fire extinguishers on 7/22/24 (attachments 5,6,7,and 8). The date of the inspection is now on the extinguishers, in addition to the 2024 tag. |
