| BrightPath Human Services responded to citation 6400.107 promptly by unplugging the electric fireplace until it could be removed from the entertainment center stand. All program staff were notified not to utilize the fire place. Director of IDD Services initiated a request to have the electric fireplace removed from the entertainment center. Maintenance removed the electric fire place from the entertainment center on 7/17/2025. Will send picture to BHSL Supervisor showing compliance with 6400.107. All homes were reviewed during this licensing by BHSL. No other programs had portable space heaters. Confirmed by Executive Director on 7/31/2025. |

Individual #1's physical examination was updated to show medical information pertinent to diagnosis and treatment in the event of an emergency. Executive Director updated the new physical form and ensured it met all regulatory requirements on 7/22/2025. Program Manager updated the annual physical to include the emergency information on 7/30/2025. All current individual physicals were reviewed by BSHL during their onsite visit. BrightPath Human Services was cited on the physical forms of 3 of 4 individuals served due to physical form missing 6400.141(c)(14). All physical forms were reviewed by executive director, no other issues noted outside of what was cited during licensing. Confirmed on 7/30/2025 |
