6400.65At 11:27AM on 5/28/2026, there was no mechanical ventilation or window in the bathroom of the basement of the home.Living areas, recreation areas, dining areas, individual bedrooms, kitchens and bathrooms shall be ventilated by at least one operable window or by mechanical ventilation. Upon receipt of the deficiency notice, BrightPath Human Services contacted a licensed plumber to evaluate options for addressing the non-compliant bathroom configuration. Following the evaluation, it was determined that the most appropriate corrective action was to remove the toilet from the room, as the area contained only a toilet and a hot water heater. The space was subsequently converted into a storage closet. This corrective action was completed on June 11, 2026, bringing the home into compliance with Regulation 6400.65. 6400.107On 7/16/26 at 3:53PM, there was an electric fireplace space heater in living room of the home, that was not permanently mounted and installed.Portable space heaters, defined as heaters that are not permanently mounted or installed, are not permitted in any room including staff rooms. BrightPath Human Services responded to citation 6400.107 promptly by unplugging the electric fireplace until it could be removed from the entertainment center stand. All program staff were notified not to utilize the fire place. Director of IDD Services initiated a request to have the electric fireplace removed from the entertainment center. Maintenance removed the electric fire place from the entertainment center on 7/17/2025. Will send picture to BHSL Supervisor showing compliance with 6400.107. All homes were reviewed during this licensing by BHSL. No other programs had portable space heaters. Confirmed by Executive Director on 7/31/2025. 08/04/2025 Implemented

BrightPath Human Services responded by updating their annual physical form to include 6400.141(c)(14). Individual #1's physical examination was updated to show medical information pertinent to diagnosis and treatment in the event of an emergency. Executive Director updated the new physical form and ensured it met all regulatory requirements on 7/22/2025. Program Manager updated the annual physical to include the emergency information on 7/30/2025. All current individual physicals were reviewed by BSHL during their onsite visit. BrightPath Human Services was cited on the physical forms of 3 of 4 individuals served due to physical form missing 6400.141(c)(14). All physical forms were reviewed by executive director, no other issues noted outside of what was cited during licensing. Confirmed on 7/30/2025 08/04/2025 Implemented
