The updated rights will be documented for each individual who resides within our community home program and their guardians, and they will be asked to sign off that they have had these rights explained to them. 6400.21(a)Direct Service Worker #1 date of hire 10/24/16 had a criminal history record check submitted on 11/4/16. Direct Service Worker #2 date of hire 10/24/16 had a criminal history record check submitted on 11/4/16. An application for a Pennsylvania criminal history record check shall be submitted to the State Police for prospective employes of the home who will have direct contact with individuals, including part-time and temporary staff persons who will have direct contact with individuals, within 5 working days after the person's date of hire. Effective 1/16/17, the human resources secretary will ensure that all application for PA criminal history check and finger printing will be submitted within the first 5 working date after the employees date of hire. The human resources secretary will develop a spread sheet to show an employee¿s name, date of hire and the date of the submission for all clearances and finger prints. If the system is not operational, at the time an attempt is made to complete the finger prints, the human resources secretary with note the date and time in the comments section of the spread sheet and re-attempt the clearances and finger prints when the system is operational or the next business day. The secretary of human resources will file all clearances in the employee¿s file within in 2 business days of receipt of the information. The Vice President of Human Resources will review the spreadsheet monthly for the first three months then quarterly thereafter to ensure compliance. 01/23/2017 Implemented
6400.151(c)(2)Direct Service Worker #3 had a Tuberculin skin test completed on 11/16/12 and then again on 10/14/16. The physical examination shall include: Tuberculin skin testing by Mantoux method with negative results every 2 years; or, if tuberculin skin test is positive, an initial chest x-ray with results noted. Tuberculin skin testing may be completed and certified in writing by a registered nurse or a licensed practical nurse instead of a licensed physician, licensed physician's assistant or certified nurse practitioner. Effective 1/16/17, all Tuberculin skin testing by Mantoux all completed tests will be filed in the employee¿s file by the human resources secretary. The date of the test will be entered into a data system and a copy of the original test will be scanned and stored electronically. The human resources secretary will complete random audits throughout the year on all employee files to ensure compliance with the regulations concerning staff physical examinations, 6400.151 (c)(2). A checklist will be completed by the human resources secretary and reviewed by the Vice President of Human Resources. Any deficient items on the checklist will be corrected by the human resources department within 2 business days, by placing a copy of the scanned and electronically stored version of the original Tuberculin Skin test results.[Immediately, the Vice president of HR, will develop and implement a tracking and notification of employee system to ensure the employee is notified and completes the Tuberculin skin testing, timely. 6400.71The telephone number for the nearest police department was not on or by the telephones in the home. Telephone numbers of the nearest hospital, police department, fire department, ambulance and poison control center shall be on or by each telephone in the home with an outside line. The telephone number for the police was added to every phone with an outside line in addition to the numbers for fire, ambulance, the nearest hospital and the poison control center. A copy of the new postings were given to the inspector prior to the end of the survey. The Community Home Managers will inspect the phones monthly and document this on the Monthly House Inspection.
