This door was equipped with a doorknob with a "privacy lock," which is a type of lock that can be opened from outside without the use of a key (in this case, the lock could be disengaged by pushing a thin object into a hole on the knob). The doorknob was installed on the door such that the locking mechanism faced the kitchen side of the door. In effect, if the lock were accidentally engaged and the door were closed while someone was in the home's basement, that person would be locked inside of the basement, impeding egress from the home in the event of a fire or other emergency. | Stairways, halls, doorways, passageways and exits from rooms and from the building shall be unobstructed.

| Article X.1007 | Staff #1 was hired by the provider effective 05/29/2025. Staff #2 was hired by the provider effective 07/02/2025. Staff #3 was hired by the provider effective 09/02/2025. The Provider is required to maintain criminal history checks and hiring policies for the hiring, retention, and utilization of staff persons in accordance with the Older Adult Protective Services Act (OAPSA) (35 P.S. § 10225.101 -- 10225.5102) and its regulations (6 Pa. Code Ch. 15). Federal Bureau of Investigation (FBI) background checks were not completed for Staff #1, Staff #2, or Staff #3, and there was no indication that these staff were residents of Pennsylvania for two years prior to their respective dates of hire. | When, after investigation, the department is satisfied that the applicant or applicants for a license are responsible persons, that the place to be used as a facility is suitable for the purpose, is appropriately equipped and that the applicant or applicants and the place to be used as a facility meet all the requirements of this act and of the applicable statutes, ordinances and regulations, it shall issue a license and shall keep a record thereof and of the application. | All three staff signed attestations that they were residents of Pennsylvania for the two years prior to employment. |
