There was no additional documentation that the course renewal requirements had been met as required. There was no documentation to support that the annual practicum had been completed as required. The annual practicum consists of two observations of medication administration and two Medication Administration Record (MAR) reviews. Observations occur in 6-month intervals based on the date the person received the initial training.
