6400.151(c)(2)The Tuberculin skin testing, completed 3/6/2020 for Direct Service Worker #1 was read by a Certified Clinical Medical Assistant. [Repeat Violation 10/24/2019] The physical examination shall include: Tuberculin skin testing by Mantoux method with negative results every 2 years; or, if tuberculin skin test is positive, an initial chest x-ray with results noted. Tuberculin skin testing may be completed and certified in writing by a registered nurse or a licensed practical nurse instead of a licensed physician, licensed physician's assistant or certified nurse practitioner. A new physical form has been created that includes the required qualified medical professionals that are permitted to administer and read a TB test. The program specialist will review all new physicals going forward to ensure the exam is completed in its entirety and TB is completed by and read only by the qualified provider. Physical exams that have already been completed, and are scheduled for subsequent TB tests will be given the new and revised physical form that includes the required components. The rights document did not include the following rights: 6400.32d, to be treated with dignity and respect; 6400.32f through 6400.32g, to choose, accept risks, refusal and control the individual's schedule; 6400.32p through 6400.32t, choose with whom to share a bedroom, furnish and decorate bedroom and common area, to lock doors in bedrooms and in the home.The home shall inform and explain individual rights and the process to report a rights violation to the individual, and persons designated by the individual, upon admission to the home and annually thereafter.Individual rights was revised to include the missing components by this program specialist.
