6400.68(b) | Hot water in the main hallway bathtub measured at approximately 124.1 degrees Fahrenheit. | Hot water temperatures in bathtubs and showers may not exceed 120°F. | On the day of discovery, the water heater temperature was set at 120 degrees. The thermostat was turned down to 110 degrees. The water temperature then measured 112 degrees from that bathtub and other faucets throughout the home all remained between 110-115 degrees. A video was provided to the inspector on 11/2/2021 confirming the safe temperature. Temperatures were rechecked one week and two weeks later and remained under 120 degrees (see Appendix A). Previous to this incident, the home staff was regularly checking water temperatures monthly in the ¿kitchen¿ and the ¿bathroom¿ at the same time as the fire drill, but these monthly checks did not designate any rotation of which specific bathrooms needed to be checked or whether or not a sink, bathtub, or shower should be checked. |

6400.166(b) | All medications prescribed to individual 4 were not logged immediately after administration on 10/31/2021 when medication administration records were checked on November 1, 2021. Per the record the following medication was not logged:
Xarelto tab 10mg 8am, levothyroxine 125mcg tab 8am, melatonin 3mg tablet 8pm, Montelukast 5mg chewable 8pm , polyeth glyc pow 3350 NF 8am, vitamin D dro 400 unit 4pm, aripiprazole tab 2mg 5pm, citalopram sol 10mg/5ml 8am, and fluticasone spr 5mcg 8pm in nostrils | The information in subsection (a)(12) and (13) shall be recorded in the medication record at the time the medication is administered. | On the day of discovery, there were missing signatures from the previous day for medication administrations for Individual 4 were confirmed to have been administered by recovering all blisterpacks from the trash. The tops of all blisterpacks had been cut off and shredded to protect privacy, but it was confirmed by the residential director that all pills were punched out and initialed for all cards and that there were the exact number of cards as number of scripts for all 3 residents of the home. Staff person signed the log to indicate the missed signatures for the previous day on 11/1/2021 when she came on shift. A MAR audit was conducted for October and no additional missing signatures or errors were discovered (Appendix F). |
